On your first visit we will need to collect a lot of information about your medical history, your symptoms, your general state of health and wellbeing, the doctors involved in you care and so on. We also need you to read and sign some paperwork. Please review the forms here. It will be great if you could fill and print them and bring them to our first meeting.
